I, the parent/legal guardian of the above-named child, hereby give permission for my child to participate in all activities, practices, games, tournaments, camps, clinics, and events organized by Golden Deers FC.

I understand that participation in soccer and related athletic activities involves inherent risks, including but not limited to falls, collisions, contact with other participants, equipment-related injuries, weather-related conditions, and other risks that may result in serious injury, illness, disability, or death. I voluntarily assume all such risks on behalf of my child.

I hereby release, waive, discharge, and hold harmless Golden Deers FC, its coaches, trainers, volunteers, staff members, directors, sponsors, facility owners, and affiliated organizations from any and all claims, liabilities, demands, actions, damages, costs, or expenses arising out of or related to any injury, illness, loss, or damage sustained by my child while participating in program activities, except where prohibited by law.

I certify that my child is in good physical condition and is capable of participating in soccer activities. I understand that it is my responsibility to inform Golden Deers FC of any medical conditions, allergies, injuries, medications, or special needs that may affect my child's participation.

In the event of an emergency, and if I cannot be reached, I authorize Golden Deers FC staff, coaches, or representatives to obtain emergency medical treatment for my child as deemed necessary. I understand that I am responsible for any medical expenses incurred as a result of such treatment.

I grant permission for Golden Deers FC to use photographs, video recordings, and other media containing my child's image for promotional, educational, or marketing purposes, including on websites, social media platforms, brochures, and other club materials, without compensation.

I understand that participation is a privilege and that Golden Deers FC reserves the right to remove any participant whose behavior is disruptive, unsafe, inappropriate, or detrimental to the program, without refund.

By signing below, I acknowledge that I have read, understood, and voluntarily agree to the terms of this Waiver and Release of Liability.
